Thank you for the opportunity to talk with you at the last meeting and to present some basic information regarding estate planning.  As promised, I am sending you the statutory form for the Utah Advanced Health Care Directive along with instructions on how to fill out the form.  Please read the instructions carefully before completing the form.  Remember, you must sign this form in the presence of one witness and a notary public. Once signed, you should provide a copy to your health care provider (usually your primary care physician) and copies to those people you name as agents.

I have also included a checklist on the basics of estate planning.  I would be happy to answer any questions you may have about your current estate plan or about the possibility of creating a customized plan for you and your family.
